1. Sum up US$13.00 per book. If you reside in New Jersey, add $0.78 per book for New Jersey sales tax. Add $5.00 to your total for shipping and handling.

3. Write the order code "BOOK 1" on the check or money order.
4. Print your complete mailing address clearly on a piece of paper.
